The immutable record of who changed what, when.

An audit log records every significant action — approvals, record changes, permission grants, sign-offs — with actor and timestamp, in a form that can't be quietly edited afterwards.

It's what makes 'who approved this purchase?' or 'who changed this isolation step?' answerable — for compliance, incident review and plain trust in the system's history.
